What should count as baseline evidence?
Depending on the priority, evidence may include classroom observations, assessment results, student work, attendance, parent feedback, staff data, behaviour patterns, admissions/retention data or implementation records. Avoid using a single metric when several sources can provide a more accurate picture.

- Confirm baseline and intended outcome.

What makes a school improvement plan fail?
- too many priorities at the same time;
- actions that are not linked to a diagnosed root cause;
- no clear owner for implementation;
- success indicators that measure activity rather than impact;
- review meetings that report progress but do not change weak actions;
- plans that sit separately from classroom practice and teacher development.
